When Is Shoulder Pain a Rotator Cuff Problem?

A clinician examining a patient’s shoulder in a clinic

Shoulder pain is worth an orthopedic visit when it lasts more than about two weeks, wakes you at night, or stops you lifting the arm, reaching overhead, or sleeping on that side. A rotator cuff problem is one common cause. The cuff is the group of tendons that keep the ball of the shoulder centred while you lift. At S&S Clinics in Manikonda, Hyderabad, Dr. Shashivardhan separates cuff irritation, a tear, frozen shoulder, and neck-related pain before anyone talks about an injection or surgery.

What the rotator cuff does

The American Academy of Orthopaedic Surgeons describes the rotator cuff as muscles and tendons that hold the shoulder joint in place and let you raise and rotate the arm. Irritation of those tendons, a partial tear, or a full tear can all feel similar at first: a dull ache on the outer shoulder that is worse at night.

Not every painful shoulder is a cuff tear. Frozen shoulder limits movement in every direction. Pain from the neck can travel into the shoulder blade. A clinic exam is what sorts those apart. An MRI is useful when the exam suggests a tear that might need repair, not as the first step for a two-day ache after a workout.

When to book, and when to go to emergency care

Book an evening visit at S&S Clinics if overhead work, combing hair, or fastening a seatbelt has been painful for more than two weeks. Bring old X-rays and a list of medicines. Clinics run Monday to Saturday, 5:00 PM to 9:00 PM, and Sunday, 10:00 AM to 1:00 PM.

Go to an emergency department the same day if the shoulder looks deformed after a fall, you cannot move the hand, or the arm is numb, pale, or cold. A sudden injury in an older adult, with immediate weakness, should not wait for the next evening slot. After the emergency team checks the joint, shoulder follow-up can continue with Dr. Shashivardhan.

  • Night pain on the outer shoulder
  • Weakness when you lift a bag or a cup
  • Stiffness that is getting worse each week
  • Pain after a fall onto the arm

What the visit covers

Dr. Shashivardhan watches how you lift, rotate, and resist pressure, and checks the neck so a pinched nerve is not treated as a shoulder tear.

  1. 1

    Tell the story

Note whether the pain started with a lift, a fall, or no clear injury, and which movements wake you.

  2. 2

    Movement tests

Strength and range show whether the cuff, the joint capsule, or the neck is the main problem.

  3. 3

Imaging only if it changes the plan

An X-ray looks for arthritis or a high-riding shoulder. An MRI is considered when a repair is a real option.

  4. 4

    A plan you can do

Most cuff irritation improves with physiotherapy and a short change in overhead work. A repair is discussed for a tear that stays weak after that trial.

How treatment is chosen in Manikonda

Physiotherapy is the main treatment for irritable tendons and for many smaller tears. Strength around the shoulder blade matters as much as the cuff itself. Injections can quiet pain enough to do those exercises. They are not a cure by themselves.

Surgery is one option when a full tear leaves the arm weak and daily tasks stay limited after a fair trial of non-surgical care. Dr. Shashivardhan explains what a repair can and cannot return. Patients from Gachibowli, Financial District, and Manikonda often use the evening clinic so a first opinion does not take a workday.

Common questions

How do I know if shoulder pain is a rotator cuff tear?

You often cannot tell from pain alone. Weakness when lifting the arm, night pain, and a story of a strain or fall raise the chance of a tear. An exam decides whether a scan is needed.

Does every rotator cuff tear need surgery?

No. Many partial tears and some full tears in less active people improve with physiotherapy. Repair is considered when weakness and night pain remain after that care.

What is frozen shoulder?

Frozen shoulder is stiffness from a tight joint capsule. Movement is limited in most directions, not only when you lift. It is treated differently from a cuff tear.

Should I get an MRI before the first visit?

No. Bring scans you already have. Dr. Shashivardhan at S&S Clinics, Manikonda, will say if an MRI would change the plan.

Can neck pain feel like shoulder pain?

Yes. A pinched nerve in the neck can ache in the shoulder blade and arm. The visit checks the neck as well as the shoulder.

How long should I wait before seeing a doctor?

A mild strain that is clearly improving can be watched for a short time. Book if pain or weakness is still limiting you after about two weeks, or sooner after a fall.

What are the shoulder clinic hours?

Monday to Saturday, 5:00 PM to 9:00 PM, and Sunday, 10:00 AM to 1:00 PM. Call +91 93900 57871 or book online.
