What Should You Do in the First 24 Hours After a Suspected Fracture?

In the first 24 hours after a suspected fracture, keep the limb still in the position you found it, support it with a sling or firm padding, and apply ice wrapped in a cloth for short periods. Do not push a bone back into place and do not ask the person to “walk it off.” Go to an emergency department the same day if the skin is broken, the limb looks bent, bleeding will not stop, or the hand or foot is numb, pale, or cold. S&S Clinics in Manikonda provides orthopedic fracture care with Dr. Shashivardhan during clinic hours, after emergency problems have been stabilized.

First steps at the scene
A fracture is a break in the bone. It can be a crack with the limb still straight, or a displaced break with an obvious bend. You cannot tell which one it is from pain alone. The American Academy of Orthopaedic Surgeons advises protecting the injured area and seeking medical care rather than testing the bone by moving it.
If you are in Manikonda, Puppalaguda, or on the way through Gachibowli, the goal for the first hour is simple: stop extra movement. Removing a ring from a swelling finger, if it slides off easily, prevents it from becoming stuck. Forcing jewelry over a swollen joint can wait for a clinician.

- 1
Stop and support
Sit or lie down. Hold the limb in the position it is already in. Use a scarf, rigid board, or folded cloth as a temporary splint that crosses the joints above and below the pain.
- 2
Cover open skin
If bone or a wound is visible, cover it with the cleanest cloth you have. Do not wash a deep wound with household liquids or push the bone under the skin.
- 3
Ice through a cloth
Apply ice or a cold pack wrapped in cloth for about 15 minutes at a time. Ice directly on skin causes burns and does not set the bone.
- 4
Get the right level of care
A deformed, open, or numb limb goes to emergency care now. A painful limb that is straight, warm, and moving its fingers or toes can be assessed urgently and then followed at the clinic.
Emergency department or S&S Clinics
S&S Clinics is open Monday to Saturday, 5:00 PM to 9:00 PM, and Sunday, 10:00 AM to 1:00 PM. It is the right place for fracture assessment and follow-up once the limb is safe. It is not the place to drive past an emergency department at noon with an open fracture because an evening appointment feels more convenient.
After an emergency team reduces a badly displaced break or treats a wound, bring the discharge note, X-rays, and the name of any temporary cast to Dr. Shashivardhan. Repeating a history from memory loses details that change the next step, such as whether a joint surface was involved.
- Open fracture or bone visible through skin: emergency care
- Limb bent, cold, pale, or numb: emergency care
- Bleeding that soaks dressings: emergency care
- Painful swelling after a fall, limb still aligned and warm: urgent orthopedic review
What the orthopedic visit covers
Dr. Shashivardhan examines circulation and nerve function before talking about the X-ray. Some breaks need only a cast or brace and repeat films. Others need alignment in a procedure room or surgery, especially around joints, in children with growth plates, or when a cast cannot hold the position. Shoulder and arm injuries are assessed the same way: what is broken, what is only bruised, and what needs a sling versus an operation.
Ask when the cast can get wet, when to move the fingers or toes, and which pain is expected. Numbness that is new, a cast that becomes too tight, or pain that rises instead of settling over the next day should be reported. Tight casts are a circulation problem, not something to tolerate until the next week.
The rest of the first day at home
Keep the limb raised above heart level when you are sitting. Move the fingers or toes if you were told that is safe, because that limits stiffness. Do not take a relative’s leftover pain medicine. Eat normally unless you have been told you may need a procedure under anesthesia, in which case fasting instructions from the treating team override general advice.
Children, older adults, and anyone on blood thinners need a lower threshold for the emergency department. A wrist that “only twisted” can still be a fracture. If you are unsure, call +91 93900 57871 during clinic hours and describe the deformity, wound, and whether the fingers are warm.
Common questions
How can I tell if a bone is broken or only sprained?
You often cannot. Bruising, swelling, and pain happen in both. An examination and an X-ray decide. Do not use the ability to move the limb as proof that it is not broken.
Should I straighten a bent arm or leg?
No. Support it as it lies and go to emergency care. Straightening a deformity at home can injure nerves, skin, and blood vessels.
Is ice or heat better on the first day?
Wrapped ice for short periods is appropriate on day one. Heat is not the first-day treatment for a fresh break.
Can I come straight to S&S Clinics with a fracture?
A stable, closed injury can be seen during clinic hours. An open fracture, a bent limb, numbness, or heavy bleeding should go to an emergency department first, then follow up in Manikonda.
Do all fractures need surgery?
No. Many heal in a cast or brace. Surgery is used when a cast cannot hold alignment, a joint surface is displaced, or the wound and bone both need treatment.
What should I bring to the fracture visit?
Photo identity, the emergency discharge summary, X-ray or CT films, and a list of medicines, especially blood thinners and diabetes tablets.
When is a cast too tight?
Increasing pain, numb or blue fingers or toes, and inability to move them are warning signs. Elevate the limb and contact the treating team the same day. Do not pad a tight cast and wait overnight.
